The Challenge:
To offer a holistic, person-centred program that supported employees of a large government agency to deal with work-related and personal life stressors. The key objective to identify and address emerging health concerns before they became severe enough to impact the employees’ wellbeing.
The Solution:
Cogent Thinking implemented a risk-based and goal orientated early intervention program, allowing individuals to access the program when they experienced early warning signs of injury or illness.
Features of the program:
- Participants can self-refer, or the employer may direct refer via phone, email, or through Cogent’s secure online platform
- Upon referral, the participant is assigned a dedicated Care Coordinator (Nurse) who is the primary point of contact. The intake call assesses physical and/or psychological challenges that are affecting their work or overall wellbeing. A detailed survey establishes a baseline health status which is re-measured at program graduation
- A personalised care plan is developed to meet health and recovery goals, for physical, psychological, or a combination. Appropriate referrals to healthcare professionals are arranged and may include General Practitioners, Physiotherapists, Exercise Physiologists, Dietitians and Psychologists.
- Goal-oriented recovery plans are flexible and evolve with the participant’s progress. Throughout the 3 to 6-month duration of the program, regular check-ins are conducted to assess the participant’s progress and ensure that the treatment remains relevant and effective
- Upon program completion, the participants are provided with options for continued care, helping to prevent relapse or the recurrence of health issues.
Results of the program:
- This program has now supported 1,000+ individuals with 70%+ of individuals presenting with mental health type concerns
- The program has had multiple iterations for vulnerable populations, and for regional and remote areas
- It has achieved a 90+ Net Promoter Score from service participants
- Positive health outcomes have been measured via the PROMIS Global-10 system, with outcomes indicating statistically significant change in health for both +5.2 for Psychological Health and +5.6 for Physical Health metrics
